one of the requirements i need to accomplish is physical examination? I went to V. Luna last week (OPD) and after some laboratory exams the physician issued a medical certificate which states that i am physically fit for commissionship, but my CDC commander told me that it is not the kind of certificate needed for the requirements. now im quite confused where can i get that physical examination clearance i may say.

Did you tried asking your CDC Commander? I think he is the one who can give you sound advise.

thank you sir jedi for your reply, i tried asking my CDC Commander for specific introduction but the only information i got is " sa V. Luna dun kinukuha yun, ang alam ko may check list pa yun after mong makuha ibigay mo kagad saken dahil nasa cavite na yung papel mo" for the past few days i am reading some of the threads here, and i encountered this "Physical Examination (Form 63-A)" but im not sure if this is what i am looking for and where in the hospital i can get it. i am on my way to V. Luna right now to try my luck.

to update this thread, i went to V.Luna kanina to try my luck, at mukhang sinuwerte ata ako, nahanap ko po yung sagot sa tanong ko pagkatapos ng paikot-ikot na pagtatanong dun. . . lesson learned, "every transaction sa ARMY starts with a formal letter"
